To meet osha requirements in the construction industry employers must provide workers with fall protection whenever they work at heights of six feet or more above a lower level 29 cfr 1926 501 b 1.

It states that every ladder way floor opening or platform shall be guarded by a standard railing with standard toe board on.
When unavoidable obstructions are encountered the minimum clearance at the obstruction may be reduced to 24 inches 61 cm provided deflector plates are installed see figure d 5 of this section.

Spiral stairs must meet the vertical clearance requirements in paragraph d 3 of this section.
They require employers to provide protection for each employee exposed to fall and falling object hazards standard 1910 28 a 1.
Osha 1910 29 b contains system requirements that employers must follow to ensure guardrail systems will protect workers from falling to lower levels.
Open roof hatches should be protected.
A minimum perpendicular distance of 30 inches 76 cm from the centerline of the steps or rungs to the nearest object on the climbing side.
1910 29 b 1 the top edge height of top rails or equivalent guardrail system members are 42 inches 107 cm plus or minus 3 inches 8 cm above the walking working surface.
Same with roof hatches.
Grab bars not required at hatch exits.
1910 25 b 3 stairs have uniform riser heights and tread depths between landings.
Guard shall be provided where the roof hatch opening is located within 10 feet 3 05m of a roof edge or open side of a walking surface and such edge or open side is located more than 30 inches 76 2cm above the floor roof or grade below.
1910 27 c 7 1910 23.
Vertical clearance above any stair tread to any overhead obstruction is at least 6 feet 8 inches 203 cm as measured from the leading edge of the tread.
1910 23 open roof hatches should be protected.
Grab bars not required at hatch exits.
